Helpful information from the OHMS Nurse's Office
         MSAD #17 policy requires that all students entering the 7th grade have a physical exam before entering school in August. Please call your doctor's office now to schedule an appointment, as these appointments fill quickly. If you don't have a doctor please call the nurse's office at the middle school for assistance in getting a physical at 207-743-5946.
       All students who wish to participate in any of the sports programs at the Middle School MUST HAVE A PHYSICAL EXAM & AN ATHLETIC CONSENT FORM (IHQ) ON FILE AT SCHOOL BEFORE PARTICIPATING. Many sports start the first week of school, please plan ahead as students will not be allowed to participate in any tryout, practice or game until the above information has been provided to the school nurse. You may drop off a copy of your child’s physical exam to the Middle School, mail it to the address below or have your doctor fax a copy to 207-739-2042. You may obtain a consent form (IHQ) from the middle school main office after August 1st.

If your child needs medicine at school, you must complete the Medication Authorization form giving your child permission to receive the medicine at school. It must also be signed by your child’s doctor. This includes any prescription medication, as well as Tylenol, Ibuprofen and any over the counter medicine. The medicine must be transported by the parent. It must be in the current prescription bottle from the pharmacy.
